How to post a ban request.

Your RGC Username:Example123
The one your reporting's RGC Username:example222
The Reason your reporting him:example(leaving)
Screenshot or Replay to be present as proof(Ingame or on channel):Showing him breaking the rules.
And the Game ID(If he abused his powers ingame):
