When you join a game on the eleos bot you get to see some information of why you are banned.
Use that information to fill in this form:
Your Battle.Net nickname:
Game name:
Admin name:
Reason:
Time of event:
Unban Card Usage:
Further details:
You'll probably get unbanned if you fill it in normally, if you have been banned for a long time.
